If your child currently has any medication in the clinic, please pick up the medication on or before the last day of school. Medication WILL NOT be sent home with children. All medication MUST be picked up by a parent or guardian - this includes over-the-counter medication. Medication not picked on by the last day of school will be disposed of according to policy.
2013-14 is around the corner!
If your son or daughter will be taking medication during the 2013-14 school year, a NEWSchool Medication Authorization form must be completed. If your son or daughter has a specific health condition, a NEWStudent Specific Health Care Plan must be completed by you and the treating physician. Both these forms can be accessed on the Fayette County Board of Education website under School Health Services - flyers and forms. The completed forms(and medication, if applicable) can be returned to the clinic at the beginning of the 2013-14 school year. Have a safe and healthful summer break!
